What is it?
Ultrasound-guided shoulder barbotage is a minimally invasive procedure used to treat calcific tendinitis of the rotator cuff, a condition where calcium deposits form within a tendon in the shoulder.
Using ultrasound guidance, a fine needle is inserted into the calcium deposit. The deposit is then broken up and washed out (“barbotage”), helping to reduce pain and improve shoulder movement.
Local anaesthetic is used to numb the area before the procedure is performed.
A corticosteroid injection is often given at the same time to reduce inflammation following the procedure.
This technique allows precise targeting of the calcium deposit and avoids the need for more invasive treatments such as surgery in many cases.
Who is it for?
This treatment may be suitable for adults with:
Calcific tendinitis of the shoulder
Persistent shoulder pain not improving with physiotherapy or medication
Painful restriction of shoulder movement
Symptoms interfering with sleep or daily activity
It may be particularly helpful for:
Patients with clearly identified calcium deposits on ultrasound or imaging
Individuals who have not responded to conservative treatment
Patients looking to avoid surgery
What will it assess?
Who performs it?
Procedures are performed by a specialist musculoskeletal (MSK) Consultant Radiologist with extensive experience in ultrasound-guided interventions.
All procedures are performed under real-time ultrasound guidance to maximise accuracy and safety.
A chaperone will be present throughout the appointment.

How do I prepare?
Please wear loose, comfortable clothing that allows access to the shoulder.
Before your appointment, it is important to let us know if you:
Take blood-thinning medication

Will I need a referral?
This service is offered on a referral-only basis. A referral from an appropriately qualified healthcare professional is required prior to booking to ensure that treatment is appropriate and forms part of a safe, structured management plan.
Referrals are accepted from physiotherapists, general practitioners (GPs), and consultant specialists.
We work closely with Physio Space, an independent physiotherapy clinic based within the same building as Cardiff Ultrasound Clinic.
If you do not already have a referral, you can book a physiotherapy assessment with Physio Space. If this procedure is clinically indicated, they can refer you directly and arrange follow-up rehabilitation as needed.
Common injection areas
What to expect after the injection
The procedure typically takes around 30–45 minutes in total
It is common to experience increased discomfort for a few days following the procedure
Pain may initially increase before improving
Gradual improvement typically occurs over several weeks
Physiotherapy is often recommended to optimise recovery.
You may be advised to reduce strenuous activity for a short period
You will be asked to remain in the clinic for approximately 20–30 minutes after the injection to ensure you feel well before leaving.
You should not drive immediately after the injection
Seek medical advice if you develop increasing redness, swelling, warmth, or worsening pain after the injection.
Many patients experience significant improvement in pain and shoulder function following treatment.
Important Information
Shoulder barbotage is a well-established treatment for calcific tendinitis and can significantly reduce pain and improve function in appropriately selected patients.
As with any procedure, there are small risks, including:
Temporary increase in pain
Bleeding
Infection
Incomplete removal of the calcium deposit
These will be discussed with you before treatment, and you will be asked to provide written consent.
