What is it?
Ultrasound-guided shoulder hydrodilatation is a minimally invasive procedure used to treat frozen shoulder (adhesive capsulitis).
The procedure involves injecting fluid into the shoulder joint capsule to gently stretch and expand it. The aim is to release the tight joint capsule, helping to improve movement and reduce stiffness.
The injection typically includes a combination of saline (to expand the capsule), local anaesthetic, and corticosteroid.
Using ultrasound guidance allows accurate placement of the injection into the joint, improving both safety and effectiveness.
Who is it for?
This treatment may be suitable for adults with:
Frozen shoulder (adhesive capsulitis)
Significant shoulder stiffness and restricted movement
Pain affecting sleep or daily activity
Symptoms not improving with physiotherapy or simple pain relief
Frozen shoulder is often a self-limiting condition, but symptoms can persist for many months without treatment.
It may be particularly helpful for:
Patients in the stiff or freezing phase of frozen shoulder
Individuals with marked restriction in movement
Patients looking to avoid more invasive procedures

Procedures are performed by a specialist musculoskeletal (MSK) Consultant Radiologist with extensive experience in ultrasound-guided interventions.
All procedures are performed under real-time ultrasound guidance to maximise accuracy and safety.
A chaperone will be present throughout the appointment.

How do I prepare?
Please wear loose, comfortable clothing that allows access to the shoulder.
Before your appointment, it is important to let us know if you:
Take blood-thinning medication
Have diabetes
Are pregnant or may be pregnant
Have any allergies
Have a current or recent infection
Will I need a referral?
This service is offered on a referral-only basis. A referral from an appropriately qualified healthcare professional is required prior to booking to ensure that treatment is appropriate and forms part of a safe, structured management plan.
Referrals are accepted from physiotherapists, general practitioners (GPs), and consultant specialists.
We work closely with Physio Space, an independent physiotherapy clinic based within the same building as Cardiff Ultrasound Clinic.
If you do not already have a referral, you can book a physiotherapy assessment with Physio Space. If this treatment is clinically indicated, they can refer you directly and arrange follow-up rehabilitation as needed.

What to expect after the injection
You will be asked to remain in the clinic for approximately 20–30 minutes after the procedure to ensure you feel well before leaving
The procedure typically takes around 20–30 minutes in total
You may experience temporary numbness for a few hours due to local anaesthetic
You may feel a sensation of pressure or fullness in the shoulder during the injection
It is common to experience discomfort for a few days following the procedure
You will be encouraged to gently move the shoulder shortly after the procedure
Improvement in movement may begin within days, with continued progress over several weeks
Physiotherapy is an important part of recovery and is strongly recommended
You should not drive immediately if shoulder movement or comfort is affected
Seek medical advice if you develop increasing redness, swelling, warmth, or worsening pain after the procedure.
Many patients experience significant improvement in shoulder movement and reduction in pain following treatment.

Hydrodilatation is a well-established treatment for frozen shoulder and can improve pain and movement in appropriately selected patients.
As with any procedure, there are small risks, including:
